Intermittent fasting is an umbrella term for eating patterns that cycle between periods of voluntary food restriction (fasting windows) and periods of normal eating, rather than restricting the type or total daily amount of food consumed continuously.

Common protocols include time-restricted eating (such as 16:8, eating only within an 8-hour daily window), alternate-day fasting (ADF, alternating between a normal-eating day and a very-low-calorie or zero-calorie day), and the 5:2 approach (five days of normal eating and two non-consecutive days of substantially reduced calories per week). These differ meaningfully from each other in how much they reduce total weekly calorie intake, which complicates comparing results across studies.
A number of randomized controlled trials have compared intermittent fasting protocols to continuous calorie restriction for weight loss and metabolic markers. A widely cited example is a 2020 study in JAMA Internal Medicine (Lowe et al.) testing 16:8 time-restricted eating against a normal eating pattern, which found no significant difference in weight loss between groups when calorie intake wasn't otherwise controlled. Broader systematic reviews and meta-analyses have generally found that intermittent fasting produces weight loss and metabolic improvements roughly comparable to continuous caloric restriction matched for total calories, rather than a unique metabolic advantage from the timing pattern itself, though some trials do show modest independent benefits on specific markers like insulin sensitivity.
Separately, intermittent fasting protocols reliably activate the fasting-related signaling changes discussed under autophagy and AMPK in animal studies, and some human studies have found time-restricted eating improves markers of metabolic health independent of weight loss. However, no human trial has demonstrated that intermittent fasting extends lifespan or prevents disease beyond what would be expected from its effects on weight and metabolic markers, so those downstream benefits should be understood as the current, evidence-supported claim, not lifespan extension itself.
